 The idea of the wisdom of crowds has been around for a while but it is gaining momentum thanks to web 2.0. Most people think this is a good thing. Take Wikipedia for example. The issue is not Wikipedia itself but the way in which the site has become regarded and used. This is a point made in an essay by Jaron Lanier.

It may be easier to raise funds for research into high-profile diseases like AIDS, malaria or TB, but money spent on this is diverted from improving health systems in poor countries. According to a WHO report, development assistance for health increased four times from $US5.6 billion in 1990 to $US21.8 billion in 2007. Until now, it was not known how that money was spent and whether it was directed effectively towards the everyday work of hospitals and clinics.

I’m trying to keep a straight face, but the National Institute for Health and Clinical Expertise (NICE) is exploring whether the medical profession should use ‘Patient important outcome measures’. It’s a bit like stepping up to reach out for significant collaborative outcomes. Seriously though, medicine has for too long worried about the effects of treatment without asking the people being treated.

Imagine a pill that goes exactly where it is needed in your body, and then tells your doctor what is happening. Called the iPill, this is already a prototype, thanks to Philips Research in the Netherlands.

In 1990, more than two thirds of $US5.6 billion spent on world health came from governments and the rest from Gates Foundation, corporate donations and other private philanthropists. By 2007, it was $US22 billion and private money from corporations and charities was greater than the total from all sources in 1990. So what is next? Sir Robert Feachem, former boss of Global Fund, believes agencies can only raise money if they prove that they spend wisely. He suggests a combination of smart fund-raising, careful spending and accurate measurement of outcomes.
